Adrenal Disorders

The adrenal glands are small glands located above each kidney. Although they are small, they play an important role in maintaining many essential functions in the body. They produce hormones involved in stress responses, blood pressure regulation, metabolism, electrolyte balance and other body processes. When the adrenal glands produce too much or too little of certain hormones, or when a growth or other condition affects the glands, it can lead to an adrenal disorder. Adrenal disorders can sometimes cause symptoms that are difficult to recognize because they may overlap with other common health conditions. Changes in blood pressure, unexplained weight changes, weakness, fatigue, changes in skin appearance and menstrual irregularities may have several possible causes. At MyLadyDoc in Bangalore, Dr. Hiba Gul can evaluate symptoms that may be associated with hormonal or adrenal problems and help coordinate appropriate investigations and specialist care when required.

What Are the Adrenal Glands?

The adrenal glands are two small glands located above the kidneys. Each adrenal gland has two main parts: the adrenal cortex, which is the outer portion, and the adrenal medulla, which is the inner portion. These areas produce different hormones. The adrenal cortex produces hormones such as cortisol, aldosterone and adrenal androgens, while the adrenal medulla produces hormones including adrenaline and noradrenaline. These hormones help regulate important functions such as the body's response to stress, blood pressure, metabolism and salt and fluid balance.

What Are Adrenal Disorders?

Adrenal disorders occur when the adrenal glands do not function normally. The problem may involve too much hormone production, too little hormone production, an abnormal growth or tumour, problems affecting the adrenal glands themselves, genetic or inherited conditions, or certain medications or other medical conditions. Some adrenal disorders are relatively uncommon, but identifying them is important because abnormal hormone levels can affect several body systems.

Common Types of Adrenal Disorders

There are several types of adrenal conditions. Some of the better-known disorders include adrenal insufficiency, Cushing syndrome, primary aldosteronism, adrenal tumours and pheochromocytoma. Each condition affects adrenal hormone production or adrenal gland function differently and may cause a different combination of symptoms. Proper medical evaluation is important because these symptoms can overlap with many other health conditions.

Adrenal Insufficiency

Adrenal insufficiency occurs when the adrenal glands do not produce enough of certain hormones, particularly cortisol. Primary adrenal insufficiency, sometimes called Addison's disease, occurs when the adrenal glands themselves are damaged or unable to function adequately. Secondary adrenal insufficiency can occur when problems involving the pituitary gland result in inadequate stimulation of the adrenal glands. Symptoms may include fatigue, muscle weakness, loss of appetite, weight loss and low blood pressure.

Cushing Syndrome

Cushing syndrome occurs when the body is exposed to excessive cortisol over a prolonged period. It can result from certain medications or from conditions that cause the body to produce too much cortisol. Possible symptoms include weight gain, particularly around the abdomen, a rounded facial appearance, thinning skin, easy bruising, muscle weakness, high blood pressure, changes in blood sugar and menstrual irregularities. The symptoms can vary considerably between individuals.

Primary Aldosteronism

Primary aldosteronism occurs when the adrenal glands produce excessive amounts of aldosterone. Aldosterone plays an important role in controlling blood pressure and the body's sodium and potassium balance. Excess aldosterone can contribute to high blood pressure and, in some cases, low potassium levels. Persistent or difficult-to-control high blood pressure may sometimes require evaluation for an underlying hormonal cause.

Adrenal Tumours

Adrenal tumours can be benign or malignant. Some adrenal growths do not produce hormones and may be discovered incidentally during imaging performed for another reason. Other tumours can produce excess hormones and cause symptoms related to cortisol, aldosterone or other adrenal hormones. The appropriate evaluation depends on the size and appearance of the growth and whether it is producing hormones.

Pheochromocytoma

Pheochromocytoma is an uncommon tumour that develops from cells of the adrenal medulla and can produce excessive amounts of adrenaline-like hormones. Possible symptoms include episodes of high blood pressure, headache, sweating, rapid heartbeat, palpitations and trembling. Because these symptoms can have several possible causes, appropriate medical evaluation is important.

What Causes Adrenal Disorders?

The causes vary depending on the specific adrenal condition. Possible causes may include autoimmune disease, genetic conditions, adrenal tumours, pituitary disorders, certain medications, adrenal gland injury or disease, and hormonal regulation problems. In some cases, the exact cause may not be immediately apparent. A detailed medical history and appropriate hormonal testing can help determine what may be affecting adrenal function.

Symptoms of Adrenal Disorders

Adrenal disorders can cause different symptoms depending on which hormone is affected. Possible symptoms include persistent fatigue, muscle weakness, unexplained weight gain or weight loss, changes in blood pressure, changes in blood sugar, changes in skin pigmentation, increased thirst or urination, menstrual irregularities, excessive hair growth, headaches, palpitations, sweating and loss of appetite. Not everyone with these symptoms has an adrenal disorder. Because many of these symptoms overlap with thyroid disorders, nutritional deficiencies, diabetes, reproductive hormone disorders and other conditions, proper evaluation is important before assuming that the adrenal glands are responsible.

Adrenal Disorders and Women's Health

Hormonal changes caused by adrenal disorders can sometimes affect women's reproductive health. Depending on the underlying condition, women may experience irregular menstrual periods, changes in menstrual flow, excess facial or body hair, acne, fertility difficulties, changes in body weight and changes in sexual health. Some adrenal conditions can influence androgen levels, which may contribute to symptoms that resemble other hormonal disorders. For this reason, women with unexplained menstrual or hormonal changes may benefit from a comprehensive evaluation rather than focusing on one hormone alone.

Adrenal Disorders and Blood Pressure

The adrenal glands play an important role in blood pressure regulation. Aldosterone helps regulate sodium, potassium and fluid balance, and excess production can contribute to high blood pressure. Certain adrenal conditions can also cause episodes of significantly elevated blood pressure along with symptoms such as headache, sweating or palpitations. If blood pressure remains unusually high despite treatment, your doctor may consider whether a hormonal cause should be investigated.

How Are Adrenal Disorders Diagnosed?

Diagnosis depends on the suspected condition. Your healthcare professional may begin by reviewing your symptoms, medical history, current medications, blood pressure, changes in weight, menstrual and reproductive history and family history. Hormonal testing may then be recommended, and depending on the suspected disorder, tests may assess levels or activity of hormones such as cortisol, aldosterone or other adrenal-related hormones. Additional imaging may also be required when a structural problem or adrenal growth is suspected, including CT scans, MRI or other imaging tests when clinically appropriate. Not every patient requires every test, and investigations should be selected according to the symptoms and suspected condition.

Adrenal Hormone Testing

Adrenal hormone testing must be interpreted carefully because hormone levels can vary depending on the time of day, medications, stress and other factors. For example, cortisol follows a daily pattern, so the timing and type of testing can be important. Your doctor may recommend blood, urine or saliva-based testing depending on the suspected condition and the clinical question. A single abnormal result does not always establish a diagnosis, and additional testing may sometimes be needed.

Treatment of Adrenal Disorders

Treatment depends entirely on the specific adrenal condition. For adrenal insufficiency, hormone replacement may be required to replace hormones that the body is unable to produce adequately. For conditions involving excessive hormone production, treatment may involve medication, surgery or other specialist-directed therapies. If an adrenal tumour is present, the treatment depends on whether it produces hormones, its size and appearance, and whether it is benign or malignant. Because adrenal disorders can be complex, treatment should be individualized rather than based only on symptoms.

Treatment for Adrenal Insufficiency

People with adrenal insufficiency may require replacement of hormones that their bodies cannot produce adequately. Treatment is usually prescribed and monitored by a healthcare professional. Patients with adrenal insufficiency may also need specific instructions about adjusting medication during illness, surgery or other situations that increase the body's need for cortisol. The treatment plan should be clearly understood because inadequate cortisol replacement during significant physical stress can be dangerous.

Treatment for Excess Hormone Production

When the adrenal glands produce excessive amounts of a hormone, treatment aims to control the hormone excess and address its underlying cause. Depending on the condition, treatment may include prescription medications, surgical removal of an abnormal adrenal gland or tumour, treatment of an underlying pituitary disorder and ongoing hormonal monitoring. The appropriate approach depends on the specific diagnosis.

Adrenal Tumour Evaluation

If an adrenal mass is discovered incidentally during an imaging scan, it does not necessarily mean that cancer is present. The medical team may assess the size of the mass, imaging characteristics, whether it produces hormones, changes over time, relevant symptoms and other risk factors. Further testing may be recommended before deciding whether observation, additional imaging, specialist review or surgery is appropriate.

Adrenal Disorders and Pregnancy

Adrenal hormone disorders require careful management during pregnancy because hormonal changes can affect both the mother and developing baby. Women with a known adrenal disorder who are planning pregnancy should discuss their condition with their healthcare team before conception when possible. Medication may need to be reviewed, and hormone levels may require monitoring throughout pregnancy. If you are pregnant and have a known adrenal condition, do not stop or change prescribed medication without medical advice.

Can Adrenal Disorders Affect Fertility?

Some adrenal hormone disorders can interfere with reproductive hormone balance. Excess androgen production, abnormal cortisol levels and other hormonal changes may contribute to irregular periods or difficulties with ovulation. However, infertility has many possible causes. If you are experiencing irregular periods or difficulty conceiving, your doctor may evaluate several hormonal and reproductive factors rather than assuming that an adrenal disorder is responsible.

When Should You See a Doctor?

Consider seeking medical evaluation if you experience persistent or unexplained symptoms such as significant unexplained weight changes, persistent fatigue or weakness, unusual changes in blood pressure, recurrent episodes of palpitations and sweating, persistent headaches associated with blood pressure changes, changes in skin pigmentation, irregular menstrual periods, new excessive facial or body hair or unexplained changes in blood sugar. Prompt medical attention is particularly important if symptoms are severe or rapidly worsening.
